नमः शिवाय शान्ताय कारणत्रय हेतवे ।
निवेदयामि चात्मानं त्वं गतिः परमेश्वर ॥

Namah Śivāya Śāntāya Kāraṇa-traya Hetave
Nivedayāmi Chātmānam Tvam Gatiḥ Parameśvara

English Meaning:

Salutations to Shiva, the embodiment of peace, the cause behind the three causes (creation, preservation, dissolution).
I surrender myself to You; You are the supreme refuge, O Lord.
